I think if you offer your service at fiver you are not on the next level. Fiverr is a page for semi-profesionel works. ;)

newsletter marketing is the actual best way. SEO also is interesting.

@ 2) I think it depends of where you are living. If you live e.g. in Germany freelancer or odesk are the wrong aim.
